This award recognises a general practitioner whose care, accessibility and leadership have made a real difference to their patients and practice. Judges look for patient outcomes, access and contribution to the practice and community.

Additional criteria
Entries should include a concise case study, supporting figures and, where possible, feedback from patients, service users, families or colleagues. Never include information that could identify a patient or service user: describe cases in anonymised terms and give figures as percentages or ranges. Nothing in an entry is shared beyond the judging panel. Judges consider the size of the service, team or organisation, so smaller providers compete on equal terms.
