About this category
This award recognises an individual healthcare professional, in any discipline, whose care, skill and commitment over the last twelve months have made an exceptional difference to patients and colleagues. Judges look for the entrant's own contribution and its effect on care.

Additional criteria
Entries should include a concise case study, supporting figures and, where possible, feedback from patients, service users, families or colleagues. Never include information that could identify a patient or service user: describe cases in anonymised terms and give figures as percentages or ranges. Nothing in an entry is shared beyond the judging panel. Judges consider the size of the service, team or organisation, so smaller providers compete on equal terms.
